Rehabilitation case management services are provided to successfully implement a Life Care Plan, individualized service plan and/or Individualized Educational Plan (IEP). Specific services may include facilitation, advocacy, coordination, education, problem solving and cost containment analysis. The goal of case management is to insure efficiency and quality of care to the disabled individual and reduce liability in fund management. Case management services are provided directly to the individual experiencing disability and also coordinated through family members, attorneys and trust officers handling fiduciary accounts.
